【 Abstract 】

Objectives: To explore the clinical curative effect of cistanche granule joint QianlieKang tablets for chronic prostatitis. Methods: 90 patients with chronic prostatitis ( CP) in our hospital from June 2015 to June 2016 were included and randomly divided into the treatment group to receive cistanche granule joint Qianlie Kang tablets and the cistanche granule control group and the QianlieKang tablet control group, each of 30 cases and all for 12 weeks. The NIH - CPSI scores, the scores of traditional Chinese medicine syndrome, EPS examination, and hemorheology examination were observed before and after treatment respectively, and the treatment efficacy was compared. Results: After treatment, all the EPS examination, the scores of traditional Chinese medicine syndrome and hemorheology in all the groups were improved and the NIH - CPSI scores significantly decreased, more significant in the treatment group than the other two control groups with a significant difference. Compared with the two control groups, the pain relief and daily life ability were improved in the treatment group. Conclusion: Joint use of cistanche granule and Qianlie Kang tablets have a good clinical curative effect, which can effectively relieve the symptom of urination and improve the patient's quality of life.

Chronic prostatitis (CP) is a common genitourinary disease in men. It is a chronic inflammation of the prostate caused by infection of pathogens such as microorganisms or other factors, which in turn causes abnormal urination, pain in the prostate area, and even pain in the prostate. Will affect the patient's mental state [1]. CP is complicated and diverse due to its etiology, course of the disease, clinical symptoms, etc., which seriously affects male sexual function and reproductive function [2].

At present, the drugs used by western medicine to treat CP mainly include antibiotics and "-receptor blockers and will focus on counseling patients' psychology. Although there are more drugs for treating CP, there is a lack of specific treatment methods, and the treatment effect is not very satisfactory. It is easy to relapse or even worsen [3]. In recent years, China has made certain progress in the research of traditional Chinese medicine in the treatment of CP [4]. TCM differentiation believes that the pathogenesis of CP can be summarized as the basic deficiency, the original deficiency, that is, kidney deficiency, Deficiency of the spleen, etc., while the real signs are damp-heat, stagnation of qi and blood stasis, etc. [5-7] Therefore, most of the clinical treatment of CP is based on promoting blood circulation and removing blood stasis.

1 Material and methods

1. 1 Clinical data

The research in this article is in compliance with the regulations of the hospital ethics committee; the collection of patient data and the experiments carried out are carried out with the patient's knowledge and consent. Ninety CP patients were from June 2015 to June 2016 in our hospital. Following the principle of randomization, single-blind, and control, they were divided into the treatment group of Cistanche Granules combined with Qianliekang and the control group of cistanche Granules. And Qianliekang control group, 30 people in each group. The three groups of patients in terms of age, course of the disease, NIH-CPSI score, a quantitative score of TCM syndromes, etc.

In terms of statistical analysis, there is no significant difference (P>0.05). See Table 1. Between sets Comparable.

1. 2 Diagnostic criteria

Western medicine diagnostic criteria: Chronic Prostatitis Symptom Index Standard (NIH-CPSI) developed by the National Institutes of Health [9]. The clinical efficacy is judged according to the NIH-CPSI score: a reduction in symptom scores <5 points are invalid, a reduction of 5 to 15 points is effective, a reduction of 15 points or more is markedly effective, and those who remain asymptomatic for more than 4 weeks without recurrence are clinically cured [10]. The total effective is counted as cured, markedly effective, and effective cases.

Diagnostic criteria of traditional Chinese medicine: National Administration of Traditional Chinese Medicine "Diagnosis and Efficacy Standards of Chinese Medicine Diseases" [11].

1. 3 Inclusion and exclusion criteria

Inclusion criteria: (1) Meet the diagnostic criteria of Western medicine and TCM; (2) Age 20-50 years old. Exclusion criteria: (1) acute prostatitis; (2) benign prostatic hyperplasia; (3) urethral stricture; (4) prostate cancer; (5) ureteral stones, varicocele, and other diseases; (6) combined with heart, brain, etc. Serious primary diseases such as liver and hematopoietic system; (7) People with allergies or allergies to multiple drugs; (8) People who are unable to cooperate, such as mental patients.

1. 4 treatment methods

The treatment group was given Cistanche Granules (Inner Mongolia Lantai Pharmaceutical Company, National Medicine Zhunzi Z20030099, batch number 20140825), 6g per bag, 1 bag/time, 3 times/d, infusion orally, 4 weeks as 1 course of treatment, continuous 3 courses of treatment; on this basis, oral Qianliekang tablets (Zhejiang Kangenbei Pharmaceutical Co., Ltd., National Medicine Standard Z33020303, batch number 20140509) 3 pieces/time, 3 times/d. In the cistanche Granules group, the course of treatment and the number of doses of cistanche Granules were taken orally in the same treatment group, and the Qianliekang group was taken orally with Qianliekang tablets.

1. 5 Evaluation Index

All patients were measured before and after treatment (1) NIH-CPSI score scale; (2) TCM syndrome changes; (3) EPS examination [12].

1.6 Data analysis

The data are expressed as mean ± standard deviation (x x ± s), and SPSS16.0 software is used for data analysis. The comparison between the means of multiple groups was performed by one-way analysis of variance, followed by a posthoc analysis with Dunnett's test, and P<0.05 was considered as a significant difference.

2 results

2.1 Comparison of NIH-CPSI scores and efficacy of the three groups of patients before and after treatment

2. 2 Comparison of quantitative scores of TCM syndromes before and after treatment in the three groups of patients

It can be seen from Table 4 that after 12 weeks of treatment in the treatment group and the two control groups, compared with this group before treatment, the quantitative scores of TCM syndromes of the patients were significantly lower than before (P <0.01), indicating that cirrhosis cistanche granules combined with Qianliekang tablets and separately

2. 3 Comparison of EPS examination data before and after treatment of the three groups of patients

From Table 5, it can be concluded that after 12 weeks of drug treatment, the EPS routine white blood cell count and lecithin changes of the three groups of patients were significantly improved (P<0.05). In addition, the curative effect of the combination of cistanche granules and qianliekang tablets is better than that of the two drugs alone (P<0.05).


3. Discussion

CP is a common disease in the male reproductive system clinically [12]. It is characterized by slowness, a long course of the disease, recurrent attacks, diverse clinical symptoms, and difficulty to cure. Affects the physical and mental health and quality of life of patients [13]. So far, modern medicine has explained. The pathogenesis of CP mainly focuses on pathogen infection, urine reflux, and immune system emergence.

Present abnormalities, lower urothelial dysfunction, psychological factors, etc. [14, 15]. Due to CP The causes and mechanisms of the disease are complex and diverse, so many scholars adopt the "snowflake model" to study CP from multiple angles, which brings new enlightenment to the treatment of CP [5]. Traditional Chinese medicine believes that CP belongs to the category of “essence and turbidity” and “cold turbidity”. Its pathogenesis mainly includes kidney deficiency as the root, damp-heat as the standard, and blood stasis as the change. The clinical syndrome can be divided into damp-heat accumulation syndrome and qi stagnation and blood stasis. Syndrome, Yin Deficiency Fire Prosperity Syndrome and Kidney Yang Deficiency Syndrome [5,16].

In this article, the use of cistanche granules to treat patients with CP has achieved good results. Cistanche Granules is a compound preparation that includes Cistanche, Morinda Officinalis, Cuscuta, Poria, Psyllium, and Schisandra. Among them, Cistanche is a monarch medicine, which has the effects of nourishing the kidney and strengthening yang, and filling the marrow; supplemented with dodder, it has the effect of nourishing the liver and kidney, strengthening the muscles and bones; Morinda Officinalis is a ministerial medicine, which can warm the kidney qi; Schisandra is an adjuvant and can exert Astringent kidney qi, astringent semen, calming the mind and soothe the nerves; in addition, Poria invigorates the spleen and oozes dampness, nourishes the heart and calms the nerves; while Psyllium is a medicine that can nourish yin and invigorate qi, and introduce medicine into the kidney [12,17, 18].

The clinical efficacy observation results show that Qianliekang and Cistanche Granules can Improve patients' NIH-CPSI, pain degree, urination symptoms, and quantitative evaluation of TCM syndromes and the therapeutic effect of Cistanche Granules is more obvious; in addition, these two drugs can also improve EPS. Studies have shown that the increase in EPS-pH is closely related to the development of CP relationships [19]. In addition, it also has a certain effect on SOD scavenging oxygen free radicals. The EPS-pH is usually higher than normal, which indicates that the prostate of CP patients is cleared from the ability of the base is reduced, and this may aggravate the inflammatory response of the prostate tissue [20-22]. It is worth noting that if the two drugs are used in combination, the effect is better than the single-use (P <0.05). These results indicate that Cistanche Granules may be a better choice for the treatment of CP.
