Abstract：

AIM： To investigate the effect of Polygonum multiflorum and cistanche compound on the immunological function of aged patients with kidney deficiency and blood stasis (KDBS) based on the pathogenesis of KDBS and the rule of tonifying the kidney and activating blood.

METHODS：①Totally 34 aged inpatients with KDBS including 22 males and 12 females with an average age of (63±3) years in the Department of Traditional Chinese Medicine, People's Hospital of Weifang were enrolled from January to December 2000. They were all informed and agreed to the treatment and detection. ②KDBS was treated with Polygonum multiflorum and cistanche compound, which was self-designed and composed of 30 g Polygonum multiflorum, 20 g cistanche, 60 g astragalus, 15 g chuanxiong, 30 g Bangui and 15 g ginseng, which was soaked for 30 minutes, and decocted with the BZE150A5 automatic decocting package machine with 500 mL water for each set, the strong fire was replaced by soft fire for 30 minutes until 200 mL, packaged into 2 bags. The patients orally took the drugs 100 mL every time, twice daily, and 6 months as one course. ③The clinical symptoms (lassitude in loin and knee, amnesia and deafness, polyuria at night, limb numbness, dizziness, and dim eyesight, insomnia and dreamful sleep, sexual disorder, purplish-red or ecchymosis tongue) were measured according to the Guideline of New Traditional Chinese Medicine in Clinical Research for Senile Disease: with the unified question, the 5-grade system (0 -4 scores) was used, 4 scores were got if patients could say the symptoms subjectively, 0 scores if the patients had no symptoms. The total score was the integral value of the clinical symptoms. ④The percentages of lymphocyte subgroup of CD3‘, CD4‘ and CD8‘ were measured by the American Elite flow cytometry before and after treatment to calculate the ratio of CD4‘ and CD8‘; the interleukin 1 (IL-1) and IL-2 levels were measured with MTT; the absolute value of lymphocyte with the SF-3000 Sysmex blood cell counter (product of Roth company); content of IgG, IgA and IgM and total sediment activity with the Array 360 protein colter (product of Beckman).⑤The t-test was used for comparison of two samples mean.

CONCLUSION: Polygonum multiflorum and cistanche compound can enhance the immunoregulatory ability and improve the clinical symptoms of elderly patients with KDBS.

Treatment method: The compound preparation of Polygonum multiflorum and Cistanche (Yishhouyin) is used to treat elderly patients with kidney deficiency and blood stasis. 30 g of angelica, 15 g of salvia, soak for 30 min, use automatic decoction packaging machine BZE150A5 (produced by Tianjin Sanyan Precision Machinery Co., Ltd., a Sino-Korean joint venture) to decoct the medicine, add 500 mL of water to each medicine, and turn to simmer for 30 minutes after boiling. min to 200 mL, packed in 2 bags. Clinical symptom scoring method: According to the "Appendix" of "Guiding Principles of Clinical Research on Geriatric Diseases Treated by New Chinese Medicines" 425: The scoring method of TCM syndromes, the clinical TCM symptoms are counted by scoring. Use uniform sentences to ask questions, and score them at 5 levels (0~4). 4 points for proactively speaking symptoms; 3 points for those with significant persistent symptoms; 2 points for those with mild or severe symptoms and intermittent symptoms; 1 point for those with mild or occasional symptoms; 0 points for those with asymptomatic symptoms. The sum of the scores is the clinical symptom score of the observed case.

Observation items: Serum samples of all patients were collected 1 day before treatment and the first day after treatment. Both groups received 5 mL of intravenous blood on an empty stomach in the morning and prepared samples according to experimental needs for determination. The American Elite flow cytometer was used to determine the lymphocyte subsets. The kit was monoclonal antibody CD4JFTTC/CD8+-PE/CD3+-PE-cy5 (product of Immunotech Opti-clone), isotype control antibody, and Optlysec red blood cell lysate. Interleukin 1 and Interleukin 2 were determined by enzyme-linked immunosorbent assay (the kit was purchased from MEDIPAN, Germany). The absolute value of lymphocytes was determined using the SF-3 000 Sysmex blood cell counter (product of Roth), and the content of immunoglobulin IgG, IgA, IgM, and total complement activity was determined using the Array360 protein reaction meter (product of Beckman).

MAIN OUTCOME MEASURES: Changes in lymphocyte subsets, interleukin-1, interleukin-2, lymphocyte absolute value, IgG, IgA, IgM content, total complement activity, and clinical symptom score before and after treatment in elderly patients with kidney deficiency and blood stasis syndrome.

Statistical analysis: The statistical processing was completed by the first author using SPSS 10.0 software. The result is expressed in the form of 5±s; the comparison of the two sample means adopts the 7 test.

2 Results

2.1 Analysis of the number of participants 34 elderly patients with kidney deficiency and blood stasis syndrome were included.

2.2 The effects of compound preparations of Polygonum multiflorum and Cistanche on the symptom score of elderly patients with kidney deficiency and blood stasis are shown in Table 1.

After treatment, most of the patient's symptom scores (except for insomnia and dreaminess) were significantly lower than before treatment (8 <0.05 ~ 0.01), indicating that the compound preparation of Polygonum multiflorum and Cistanche can significantly improve clinical symptoms.

2.4 Adverse events and side effects In clinical application, 3 cases showed loose stools after taking the medicine, and no adverse reactions were found. 2 cases showed a transient increase in blood pressure.

3 Discussion

Traditional Chinese medicine believes that the kidney is the foundation of the innate, and the essence of the kidney, which is the essence of the yin and yang, is the foundation of all life movements. Recent studies have found that the kidney is a natural process of weakness, manifested in the formation of kidney essence, blood, and qi deficiency and stasis. Interleukin 1 is one of the most important lymphokines identified in recent years and has a wide range of immunomodulatory activities. Interleukin 2 can maintain the long-term growth of T lymphocytes in vitro, activate T and NK cells, promote the proliferation and functional expression of lymphocytes, and is an indispensable lymph factor for the proliferation and differentiation of T lymphocytes. The functions of cells, NK cells, etc. have a regulatory role. Modern studies have shown that elderly organisms and patients with kidney deficiency have symptoms of low immune function. T lymphocyte function is most obviously impaired in immune aging. The reduction of interleukin 1 and 2 production is currently considered to be an important molecular mechanism of immune aging. In addition, under normal circumstances, the lymphocyte populations cooperate with each other to maintain the body's normal physiological and immune functions. When the number and function of different subgroups are abnormal, the body may cause immune disorders and a series of pathological phenomena. Modern studies have confirmed that human T lymphocytes have a regulatory effect on the type and intensity of all humoral and cellular immune responses in the body, among which CD3, CD4, and CD8 cells play a major role. CD4 can induce and assist cytotoxic T cell precursors to mature into cytotoxic T cells and enhance/cell antibody production. CD8 cells play a negative role in the immune response through their own inhibitory factors, inhibiting the function of CD4 cells and B cells, thereby inhibiting antibody formation and cellular immune response. In recent years, the concept of the ratio of CD4+ to CD8+ has been paid special attention. Data shows that the percentage of CD4 in elderly patients with chronic diseases has decreased, while the percentage of CD8 has increased, resulting in a significant decrease in the ratio of CD4: to CD8. This is consistent with the view that elderly patients with chronic diseases may be related to autoimmunity. This fully shows that the immune function of elderly patients and patients with kidney deficiency is closely related to the changes of T lymphocyte subsets, not only with the number but also with the abnormal regulation function. This experiment found that the patients were treated with the compound preparation of Polygonum multiflorum and Cistanche T lymphocyte subsets, interleukin 1, interleukin 2, immunoglobulin, total complement activity, and absolute lymphocyte values were significantly improved compared to before treatment (! <0.05+0.01), suggesting that the compound preparation of Polygonum multiflorum and Cistanche It can indeed promote the proliferation of T lymphocytes and produce interleukin 1 and interleukin 2, thereby improving immune regulation and delaying aging.
