Exploration of Medication Regularity of CUI Xue-Jiao in Treatment of Impotence

Abstract:
Objective: To explore the medication regularity of Professor CUI Xue-Jiao in treating impotence based on the Traditional Chinese Medicine Inheritance Support Platform System. Methods Medical record data of the effective cases of impotence patients treated by Professor CUI Xue- Jiao from May 2017 to May 2019 in the outpatient department of surgery(andrology)of the First Affiliated Hospital of Guangzhou University of Chinese Medicine was collected. The medication frequencies of the herbs, the properties, and tastes as well as the action classification of the enrolled herbs were calculated. Moreover, the prescription regularity and candidate recipes for impotence were mined. Results The prescriptions for 138 patients were screened and analyzed, and a total of 116 herbs were used, the accumulative medication frequency being 5 593. The top 3 herbs used frequently were Herba Epimedii, Radix Morindae Officinalis, and Herba Cistanches. The properties of the enrolled herbs were dominated by warm and neutral properties, the tastes of the herbs were dominated by sweet, pungent, and salty flavors, and the leading categories after-action classification were tonics, astringents, and herbs for activating blood to resolve stasis. After association rule analysis and cluster analysis, 15 core herbal combinations and 5 new candidate recipes for impotence were obtained. Conclusion Professor CUI Xue- Jiao believes that deficiency of liver and kidney, and Qi stagnation and blood stasis are the fundamental pathogenesis of impotence, and impotence can be mainly treated by the herbs for warming kidney and tonifying liver assisting by herbs for activating blood to resolve stasis and unblock meridians.

Impotence refers to a disease in which the penis is impotent, weak, or firm so that it cannot maintain normal sexual life. With the acceleration of modern life, changes in living environment, changes in diet structure, and increased mental stress, the incidence rate of impotence has been increasing, and the population tends to be younger. This has brought bad effects to the communication, life, and spirit of male compatriots. Before the Ming and Qing Dynasties, many doctors believed that impotence was related to kidney deficiency, mainly tonifying the kidney; Since the Ming Dynasty, many doctors have understood impotence from different angles. They believe that depression of fire, damp heat, and emotion are important factors in the pathogenesis of impotence; In the Qing Dynasty, doctors proposed that the syndrome differentiation of impotence should first distinguish between deficiency and reality, not blindly tonic, and diagnose and treat impotence by soothing the liver and regulating emotion. Professor Cui Xuejiao is the third batch of famous old experts in traditional Chinese medicine, the instructor of academic experience inheritance, the professor and doctoral supervisor of Guangzhou University of traditional Chinese medicine. Professor Cui Xuejiao is unique in the treatment of impotence. This study uses a data mining method to analyze the medication characteristics of Professor Cui Xuejiao in the treatment of impotence, so as to provide the basis for clinical treatment of impotence.

Data and methods
Collect the medical record data of effective cases of impotence diagnosed and treated by Professor Cui Xuejiao in the surgery (andrology) clinic of the First Affiliated Hospital of Guangzhou University of traditional Chinese medicine from May 2017 to May 2019.
This study mainly uses the "traditional Chinese Medicine Inheritance auxiliary system" to make statistics on the medication frequency and efficacy classification of the collected data, analyze the prescription law, excavate new drug prescriptions, etc.
Result
After screening, the prescriptions of 138 patients were collected, and 116 traditional Chinese medicines have been used a total of 5593 times. There are 15 drugs whose frequency is higher than 120 times, of which the first three drugs are epimedium, Morinda officinalis, and Cistanche deserticola.
The function of "statistics of sex, caste and meridian tropism" in the "statistical report" module of the "traditional Chinese Medicine Inheritance auxiliary platform" was used to count the traditional Chinese medicine used in the prescriptions of 138 patients. The results showed that gentle and smooth drugs were mainly used. The frequency of gentle and smooth drugs was 2584 (53.8%) and 1361 (28.3%) respectively; The frequencies of cold, hot, and cold were 799 times (16.6%), 47 times (1.0%), and 10 times (0.2%). Sweet drugs were the most delicious, with a total of 3514 times, accounting for 45.3% of the total drug frequency; Pungent drugs ranked second, with a frequency of 1404 times, accounting for 18.1%; Salty medicine 1111 times, accounting for 14.3%. Bitter drugs, sour drugs, and astringent drugs were used less frequently, 722, 598, and 403 times respectively, accounting for 9.3%, 7.7%, and 5.2% respectively.
116 traditional Chinese medicines used in the prescriptions of 138 patients were classified into 10 categories. According to the frequency of medication, the top three were tonic drugs, astringent drugs, and drugs for promoting blood circulation and removing blood stasis.
Discuss

The results of this study show that the drugs selected by Professor Cui Xuejiao for the treatment of impotence are mainly gentle and smooth. It can play different effects by cooperating with other traditional Chinese medicine. Professor Cui Xuejiao's medicine for impotence mainly tastes sweet, pungent, and salty. Sweet medicine has the functions of tonifying, harmonizing drug properties, and relieving pain; Pungent medicine has the function of dispersing and dredging Qi and blood. It is mostly used for symptoms of Qi and blood block; Salty medicine has the functions of diarrhea, defecation, softening hardness, and dispersing knots. Professor Cui Xuejiao's treatment of impotence mostly adopts the methods of Warming Yang and tonifying the kidney, activating blood circulation, and removing blood stasis. Warm drugs and sweet drugs are partial to supplement. If the dosage is too much, it is easy to affect the operation of the Qi machine. Adding spicy drugs can strengthen the role of Qi and blood circulation and regulate the Qi machine.
The statistical results of medication frequency showed that the top three medication frequencies were epimedium, Morinda officinalis, and Cistanche deserticola. Epimedium tastes pungent, sweet, and warm in nature. It can tonify the kidney, strengthen Yang, dispel wind and dehumidification. Morinda officinalis has a pungent and sweet taste and warm nature. It has the effects of Tonifying the kidney, strengthening yang, dispelling wind, and dehumidification. Cistanche deserticola tastes sweet, sour, and salty. It is warm in nature, tonifying the kidney and helping Yang, moistening the intestines, and defecating. These drugs have the function of Tonifying the kidney and helping Yang, reflecting Professor Cui Xuejiao's therapeutic principle of warming and tonifying the kidney yang. In the specific clinical syndrome, it is also necessary to add and subtract the syndrome in combination with the patient's signs and symptoms.
The statistical results of drug efficacy frequency show that Professor Cui Xuejiao mostly uses tonic drugs, astringent drugs, and drugs for promoting blood circulation and removing blood stasis in the treatment of impotence. In addition, astringent drugs and drugs for promoting blood circulation and removing blood stasis are also used more frequently. According to many years of clinical experience, Professor Cui Xuejiao often treats impotence from four syndrome types: kidney qi deficiency, kidney yang deficiency, kidney yin deficiency, and liver qi stagnation. However, in clinical practice, the symptoms complained by patients are often not as simple as the type of appeal syndrome and are often mixed with more or less other different symptoms, which affect the development of the disease. For these concurrent symptoms, it is necessary to cooperate with other drugs to treat impotence.

Examples of medical records
The patient, Mr. Li, male, 40 years old, was hospitalized on December 20, 2018, due to difficulty in erecting. The patient has difficulty erecting and will feel pain when erecting. It is treated by Tonifying the kidney and activating blood circulation. Prescription: mainly Cistanche deserticola, with Morinda officinalis, white peony, Ilex pubescent, and Sophora japonica. The prescription is 14 doses in total, 1 dose per day, decocted to 200 ml and taken warm.
Second diagnosis on January 10, 2019: the erectile condition is the same as before, the feeling of chest flatulence becomes weaker, the sleep quality becomes better, and still feels pain.
On January 25, 2019, he had three consultations and said that his symptoms were better than before. He continued to take 10 doses of the prescription. After telephone follow-up, the symptoms were improved and life was normal.
To sum up, this study studies the drug law of Professor Cui Xuejiao's clinical treatment of impotence through data mining and preliminarily reveals the general drug characteristics of Professor Cui's clinical symptoms of impotence. Because the course of impotence patients is relatively long, and the deficiency of liver and kidney is easy to cause qi stagnation and blood stasis, we should keep activating qi and blood while tonifying kidney and Yang. Cistanche deserticola plays a great role in efficacy.
