Premature ejaculation (PE) belongs to the category of sexual dysfunction. There is currently no recognized standard for its definition because its diagnosis and treatment need to consider the sexual satisfaction of the sexual partner. Most premature ejaculation is primary premature ejaculation and lasts for life, so premature ejaculation has a greater impact on couples' lives than erectile dysfunction, and there is no best cure method yet. There are many ways or methods for the treatment of premature ejaculation, but the treatment effect is uncertain. At present, everyone agrees that the combination of drugs and other comprehensive treatments on the basis of behavioral therapy has a better effect. The outpatient department of our hospital admitted 40 cases of premature ejaculation patients with kidney deficiency as the main symptom. The combined treatment of cistanche granules and tamsulosin hydrochloride (tamsulosin) has achieved remarkable results. The report is as follows.

1 Material and methods

1.1 General information:

40 cases of premature ejaculation patients were treated with Cistanche Granules and tamsulosin hydrochloride in the Andrology Clinic of our hospital, aged 18-58 years old, average age 38 years old, course of disease 1 month to 10 years, 9 cases of extravaginal ejaculation, Others will leak in less than 2min in the vagina.

1.2 Inclusion criteria:

① It occurred before the penis was inserted into the vagina or less than 2 minutes after the penis was inserted into the vagina, and the ejaculation was not controlled, which was significantly earlier than one's own wishes; ②Have a fixed sexual partner or have sex in a safe environment; ③I feel painful and have Obviously the relationship between sexual partners is tense, and the frequency of sexual partner satisfaction is less than 50%; ④Exclude genitourinary malformations, infections, trauma history, etc.; ⑤According to TCM syndrome differentiation, it is kidney deficiency type.

1.3 Treatment method:

40 patients used Cistanche Granules (Inner Mongolia Lantai Pharmaceutical Co., Ltd., batch number: Zhunzi Z20030099), 2g/time, 3 times/d, combined with tamsulosin hydrochloride (Kunming Jida Pharmaceutical Co., Ltd.) 0.4mg, once every night before going to bed for a total of 2 months. At the same time routine psychological counseling and sexual life skills guidance.

Observation indicators: After the course of treatment, under the guidance of the clinician, the patient and his sexual partner will fill in the form together, which includes erectile function, ejaculation latency, sexual satisfaction, etc.

After the treatment, the patient's sexual life improved significantly, including the ejaculation latency, sexual satisfaction, and sexual partner's sexual satisfaction. The incubation period of intravaginal ejaculation was about 5 minutes, which was improved compared to before treatment, and the difference was statistically significant.

3 Discussion

Premature ejaculation is a common sexual dysfunction in men. Because premature ejaculation seriously affects the quality of sexual life of patients and their sexual partners, women are not satisfied in their sexual life and have a greater impact on emotions and interpersonal relationships than men. Therefore, sexual partners must be involved in the course of treatment and evaluation of curative effects.

At present, the mechanism of premature ejaculation is unclear, and there is no best treatment. At present, there are many methods to treat premature ejaculation, including behavioral therapy, drug therapy, and dorsal penile neurotomy. Modern medicine believes that the disease is related to glans sensitivity, mental stress, etc., and anti-anxiety, antidepressant, sedative drugs, even oral α-receptor blockers, and external application of anesthetics to delay neurological response are often used [1]. Traditional Chinese medicine believes that premature ejaculation is mostly related to kidney deficiency, and it is not possible to capture and control sperm. In clinical practice, the method of invigorating the kidney and capturing sperm is often used. Many experts and scholars believe that the ejaculatory duct, prostate, posterior urethral tension, and other factors are indispensable links in the ejaculation process and are closely related to the occurrence of premature ejaculation. Therefore, the treatment method of combining Chinese medicine to invigorate the kidney and reduce the tension of the ejaculation duct is widely used with significant curative effects and few side effects. Tamsulosin hydrochloride is a highly selective and long-acting α1 receptor blocker, which mainly acts to inhibit the sympathetic nervous system of the ejaculation center, relax the smooth muscles of the posterior urethra and other parts, reduce the pressure, thereby prolonging the ejaculation latency [2-3 ].

Traditional Chinese medicine believes that the kidney is the foundation of sealing, and the secretion of essence is controlled by the kidney. Morinda Officinalis has the characteristics of Xingan lukewarm, enters the liver and kidney meridian, has the effects of nourishing kidney yang, strengthening muscles and bones, and dispelling rheumatism. Pharmacological studies have found that Morinda Officinalis has similar corticosteroids and hypotensive effects, which can increase hypothalamic-pituitary and ovarian stimulation. The function of Huang is clinically used for impotence due to insufficient kidney yang, uterine cold and infertility, and weakness of the waist and knees due to insufficient liver and kidney. The acid temperature of Schisandra enters the lungs and kidney meridian has the effects of restraining the lungs and kidneys, producing fluids and sweating, astringent essence, relieving diarrhea, calming the heart and soothing the mind, and can improve the symptoms of sexual dysfunction such as premature ejaculation, nocturnal emission, and slippery sperm. Studies have shown that Cistanche Yishen Granules can enhance the body's stress ability, improve the patient's libido and response to sexual stimulation, increase sexual pleasure, help control semen intake, and have a synergistic effect with tamsulosin, thereby improving the efficacy.

For the treatment of premature ejaculation, a single measure is not effective. Therefore, clinical treatments are often used in combination with psychological treatment and encouraging couples of patients to adhere to behavioral therapy, as well as comprehensive or combined drug treatment. Traditional Chinese medicine can be used for syndrome differentiation and treatment, combined with highly selective α-receptor blockers. It is currently treated with wide clinical application, simple method, convenient use, and relatively reliable curative effect, and it is worthy of promotion.
